Five Care provides person-centred support for adults with learning disabilities, autism, physical disabilities in Hertfordshire and areas within the M25 corridor in Greater London. We will strive to transition many of our clients, to re-integrate and be part of their communities, empowering and enabling them to lead the lives they choose, even those with the most complex needs and challenging behaviour.

Our Support

Our care team is fully trained on The Oliver McGowan Mandatory Training on Learning Disability and Autism to be able to provide the best possible person-centred service to our clients.

Home Care & Live-In Care

As a private care service, we are able to offer a more flexible service to your needs or the needs of the person you care about. Five Care was created to support people to live as full a life as possible at home, even though they may need a little extra support with some parts of their daily lives. We encourage independent living by putting our clients at the centre of all that we do.

From our personal experience, we understand that some people just need that little bit of support to stay at home and live just as they always have done. There are so many different things a person may want or need for their care at home. Our care package times start from just 3.0 hours per week (60 mins per day) up to full live in care (24 hours per day, 7 days per week).

Learning Disabilities

We understand and appreciate that people with a learning disability can face difficulties that others do not. This can range from everyday tasks such as managing household chores to budgeting and socialising. Particularly with socialising this can come down to a lack of understanding about how to do it in a safe and socially acceptable manner. We will support you every step of the way, no matter what barriers or hurdles there are to overcome, we will always be alongside you when you need a supporting hand.

We want to see you succeed and achieve things- simple things and things you never thought possible. Whatever the activity, whatever the need or motivation, whether it is something at home, within your community, education, or employment; we will always be behind you and committed to providing exceptional care and support so that you can achieve your dreams.

We will work in partnership with you and the other people within your support network to make sure your support plan and the support given by our staff is inclusive and specific to your needs. Where your needs, goals, or aspirations change, we will change along with them, ensuring that our staff are given specific learning disability training and all the tools to support you to succeed.We will strive to appoint support workers who match your interests in order to achieve the best outcomes, along with allowing you or your loved one to live a fulfilling life.

Autism

Our support for autistic people is designed around your needs, interests, and preferences, so we are experts in supporting you to live the life you want – where, how and with whom you choose.

We are committed to providing exceptional specialist autism care and support for you, we will work with you to create your support plan and involve you in choosing your support workers, providing you with specialist staff who understand you and your autism.

Home care enables a person with Autism to maintain positive relationships, fosters confidence in social situations and provides a consistent structure and routines, whilst providing an opportunity for family members to have a respite from the caring role.

We understand that autism has different meanings for everyone. It can range in complexity and provide people with their own strengths, difficulties, abilities and preferences. Therefore, we provide packages of support that are bespoke to each person. This approach, known as person-centred care, enables us to cater to the unique needs of each person we support.

Physical Disabilities

Living with a physical disability should not prevent you from leading a full and active life. Whether an individual’s physical disability is recently diagnosed or is something they’ve lived with for many years, our care and support are designed to ensure they can live as independently as possible.

At Five Care, we’re here to provide tailored help for disabled adults. We understand that everyone’s circumstances, conditions and preferences are going to be different. We never use a one-size-fits-all approach. We believe in working in partnership with individuals and their families. Our approach to physical disabilities support is to place customers at the heart of the support plan, and work with them to understand their needs and preferences, as well as their goals and ambitions.

We aid those who have lived with their disability for years and want the encouragement to pursue a goal they’ve wanted to do for years. We also provide support to those who have recently been diagnosed and are looking for guidance and coping strategies in everyday life.
